Our team works with every individual and their ACC case worker to ensure they get the right service for them.

Support needs assessments

Location: Nation-wide

APM delivers Support needs assessments to work out the level of support you need.

After the assessment, and depending on your needs, you can get help from a range of professionals including nurses, occupational therapists and social workers.

The aim is to help you recover from injury, stay independent and maintain your quality of life.

Our needs assessments can help if you:

  • have a physical or intellectual injury or disability
  • have experienced an injury and are getting help from ACC
  • are over 65 and need extra support with your day-to-day life

Social rehabilitation needs assessments

Location: Auckland, Waikato, Bay of Plenty, Lower North Island, South Island

These assessments help you live independently and safely by identifying the support you need after an injury.

There are two types of assessments:

  • Integrated rehabilitation assessments: look at a range of issues and determine the support that will suit you best
  • Single discipline assessments: a more specific assessment that looks at one particular issue

Housing modification assessments

Location: Waikato, Bay of Plenty, South Island

Our consultants assess your home and work with your ACC case worker and other parties to identify any adjustments such as rails, ramps or bathroom modifications that will make your daily life easier.

Wheelchair and seating assessments

Location: Waikato, Bay of Plenty, Nelson, Marlborough, Canterbury & West Coast

If you need help with mobility, our consultants assess your needs and make recommendations to ACC about suitable support.

This looks at your living and sleeping arrangements and may include trials of wheelchairs and other equipment.

Concussion services

Location: Lower North Island, South Island

We work with doctors, community medical teams and ACC to deliver the concussion service.

Every brain injury can be different and we make sure your assessment, rehabilitation and recovery needs adapt to your needs.

As part of a larger team dedicated to your health, we work together get you safely back to your day to day life and keep your independence.

Training for independence

Location: Nation-wide

Training for independence is a group of services to help meet the needs of a wide range of clients.

We provide access to education, support, rehabilitation and other services to help restore or maintain your health and ability to take part in your wider community.

We aim to reduce your need for further rehabilitation.
