When you refer a client to our team, you can be assured we will provide the best possible service to reduce the severity and impact of pain.
We take a personal approach to every client and work with every party to ensure they receive a comprehensive service to address their specific needs.
If you’re a doctor or healthcare professional you can refer your client to us directly. We will then work with ACC and you about their service.
How to refer to APM
Download and complete ACC's Örebro musculoskeletal pain screening questionnaire.
Assess your client’s score using this scoring guide.
If the score is 50 or more, complete ACC’s provider referral for pain management form ACC6273.
Email the questionnaire and ACC provider referral form to NZPainServices@apmworkcare.co.nz
We will confirm receipt of your referral and keep you informed of the progress with ACC.
